Premier/Premier Choice health plans

How does the Anthem Premier Plan work?
Premier plans are Preferred Provider Organizations (PPOs), which gives you the greatest flexibility in choosing a provider.
- Each plan has an annual deductible that must be met before the plan coverage begins
- Once the deductible is met, the Premier plan pays a portion of covered medical expenses, and you pay a percentage of coinsurance.
- Members save money on healthcare expenses when they use a provider from the PPO network.
- The plan also offers built-in generic prescription drug coverage before the deductible is met, and a smaller deductible for brand-name prescription drugs. For clients who prefer the best prescription coverage available on the market today, Anthem offers an upgrade which eliminates the Rx deductible and lessens the copayment for the brand-name tiers.
The Lumenos Plan
The Lumenos Plan from Anthem Blue Cross & Blue Shield is compatible with a tax-advantaged Health Savings Account (HSA), which allows you to make tax-free contributions to pay for qualified medical expenses. The beauty of these plans is how much easier it is to keep track of pre-deductible health spending, and how the funds can be rolled over from one year to the next.
The Lumenos Plan includes:
- Health Savings Account
- Extremely wide choice of doctors, and discounts for using in-network providers
- 100% coverage for preventive care services before your deductible is met, and no out-of-pocket payments when you see a network provider
- Once the annual deductible has been satisfied, Lumenos health plans cover all remaining expenses at 100%
- Your total out-of-pocket healthcare expenses are limited to a maximum dollar amount per calendar year, after which 100% of covered services are paid by the plan
